Please download and read carefully the
WELCOME HANDBOOK prior to your first appointment. You will
be asked to sign acknowledging that you have read and understand it's content. Please download, print, and complete with BLACK INK the INTAKE PACKET prior to your first appointment. You will need to come 15 minutes early
prior to your appointment to complete it if you do not bring it with you. Please download and complete a RELEASE OF INFO for each person or source that will be involved in your counseling. Please download and complete a CHILDREN'S ADDENDUM for clients who are under the age of 18. |
