We are military ONE SOURCE providers.  OUT OF NETWORK BENEFITS: Many policies have out of network benefits. When you have out of network benefits, you pay Life Designs for the visit. We file your insurance non-assigned and the payment should come to you. We will be glad to do that anytime. There are several insurance companies that make it almost impossible to receive payments, that we had to make the decision to either not contract with them, or not accept new clients.  We are very sorry for this inconvenience.  WE DO NOT particiate with the following Insurance Companies or they will not allow us in network. Payment Policy	Deductibles, copayments and the cost of service is collected at the time of service. If this is not possible your appointment will be rescheduled to a date that will make it possible.  We accept checks, money orders, and cash. We do not have ability to accept credit/debit cards. ALL COURT ORDERED TREATMENT IS CASH ONLY
Appointment Policy	Cancellations or rescheduling of appointments must be done 24 hours in advance unless there is an acute illness, death or, dangerous traveling conditions. If you fail to show for or cancel late not due to the reasons above, you will be responsible for the full cost of that session. It will not be covered by your insurance. This charge must be either paid in full prior to the next appointment or a payment plan can be set up.  Otherwise, your appointments will be rescheduled or cancelled. Additions to this policy is furnished at time of service.
